The Community Health Coach supports and empowers residents through personalised health coaching to embrace healthy lifestyle, manage their chronic disease conditions and improve quality of life. This includes working with residents on following-up with their health plan developed by primary care providers and community nurses, and facilitating their active participation in health-related programmes organised by community partners, such as Active Ageing Centres. He/she shall participate and organise health education programmes and community health screening events.
Diploma/Advance Diploma qualification or certifications relevant to healthcare industry is required. Has working experience in the healthcare industry is an advantage. Good knowledge of various community services and support in the community setting will be an advantage. Able to work in a team and independently. Strong communication and social engagement capabilities especially with older adults in the community. Strong organizational, coordination, and interpersonal skills. Proficient in computer skills and Microsoft Office. Able to converse in local languages is preferred.
