Social Worker (YDC)

Conduct outreach to at-risk youth, especially those with adverse childhood experiences. Provide end-to-end case management: assess, plan, intervene, review. Lead internal case discussions and coordinate with external agencies. Build strong networks in the community to improve access to support. Assess safety risks and develop safety plans for clients, families, and volunteers. Handle crisis situations with appropriate de-escalation strategies. Advocate for client needs on formal platforms to improve service delivery. Maintain accurate, timely case documentation. Recruit and engage volunteers for outreach and group facilitation roles. Screen, assess, and prepare volunteers for working with target groups. Design and conduct volunteer training to equip them with required skills. Foster partnerships internally and externally to optimise resource use and support systems. Support programme development and evaluation as needed. Plan and lead group work sessions for youths and caregivers. Maintain up-to-date professional knowledge, ethics, and best practices. Stay informed on trends in social services and emerging client needs. Write/edit reports and proposals as required. Contribute strategic insights to YDC planning with the Centre Manager. Assist in orientation, training, and appraisal processes when needed. Ensure achievement of programme KPIs and accurate reporting to funders. Work closely with the Admin Assistant for proper documentation and reporting. Perform additional duties as assigned in line with the role.
