We are looking for a passionate and enthusiastic team player to join us as a social worker who is interested in making a difference in caring for our residents and clients. Conduct psycho-social and financial assessments / reviews. Recommend and carry out appropriate clinical interventions and case management for residents and their families. Facilitate family conferences and attend multi-disciplinary team meetings. Experienced with managing the Integrated Referral Management System and the ILTC portal subvention and subsidy matters. Good interperesonal, organisation and communication skills. Competent administrative and IT skills in maintaining updated documentation of case files and time reviews of financial assistance.
