We are seeking a compassionate and organised Case Manager Assistant to support our Psychiatric Rehab team in delivering holistic, person-centred care to individuals with mental health conditions. This role involves assisting in assessments, care coordination, and psychosocial support to promote recovery and community integration. •Assist Centre Manager/Medical Social Worker/Psychologist in conducting client assessments and developing care plans. •Support clients and caregivers with psychosocial interventions and community resources. •Coordinate referrals and follow-ups with healthcare providers and social service agencies. •Participate in home visits, community outreach, and multidisciplinary team meetings. •Maintain accurate documentation and contribute to service audits and reviews.
•Diploma or Degree in Social Work, Psychology, Nursing, Counselling, or related fields. •Registration with relevant professional bodies (e.g., Singapore Nursing Board, Allied Health Professionals Council, Singapore Association of Social Workers) is preferred. •Minimum 1–3 years of experience in mental health, geriatric, or community care settings. •Familiarity with psychiatric rehabilitation principles is advantageous. •Strong interpersonal and communication skills, with a commitment to client confidentiality and dignity.
