The Medical Social Worker (MSW) provides essential psychosocial support to residents with psychiatric conditions, facilitating their recovery and reintegration into the community. This role includes conducting assessments, offering individual counselling, providing financial guidance, and coordinating access to community resources. The MSW works closely with residents, families, and a multidisciplinary team to ensure comprehensive care. The position also involves developing and maintaining Standard Operating Procedures (SOPs) to uphold service quality and consistency. •Conduct psychosocial assessments and develop individualized care plans. •Provide individual counselling to residents to address emotional, behavioural, and social challenges. •Offer financial counselling, including budgeting support, financial aid applications, and referrals to relevant agencies. •Facilitate discharge planning and community reintegration. •Coordinate referrals for housing, employment, healthcare, and social services. •Advocate for residents’ rights and access to services. •Collaborate with psychiatrists, psychologists, job coaches and occupational therapists to ensure integrated care. •Develop and maintain SOPs related to social work practices, financial counselling, and documentation. •Maintain accurate and confidential records in compliance with regulatory standards. •Participate in case conferences, team meetings, and quality improvement initiatives
