Assisting with planning, implementation, and management of community health projects. Analyzing data related to community health initiatives to identify trends and inform decision-making. Working with various community partners, healthcare teams, and patients/ caregivers to improve healthcare delivery. Providing information to patients and caregivers, often through a hotline or other channels. Performing secretariat work for meetings, managing office budget, and maintaining records. Analyzing assessment information to prioritize care needs and develop care plans. Interacting effectively with patients, healthcare professionals, and community partners.
