The Primary Care Coordinator is responsible for providing primary care coordination and administration support to facilitate care of patients. The job includes but is not limited to the following: Providing support to patients by helping to schedule appointments for ancillary services. Populating the Chronic Disease Registry (CDR), as well as updating and maintaining patient records. Tracking of patient's appointment and conditions. Providing assistance to Specialist Outpatient Clinic (SOC) clinicians in explaining available options for follow-up at primary care. Counselling and motivating patients in following up their care in primary care. Gathering feedback and conducting patient satisfaction survey.
Minimum GCE O Levels with 1 year of working experience Diploma in any discipline with no working experience can be considered Proficient in Microsoft office (Word, Excel, PowerPoint) Caring and patient-oriented
