Handle initial I&R enquiries & perform preliminary or initial assessment for programme as needed. Manage any financial needs and perform necessary financial assessments to determine necessary applications as needed. Conduct comprehensive biopsychosocial assessments and formulate intervention plans to promote clients’ independence, integration, and participation in community life. Manage a caseload across varying risk levels, applying critical thinking and clinical judgment in intervention planning. Coordinate and facilitate referrals to appropriate services and community resources, ensuring continuity of care. Provide trauma-informed, person-centered, and strengths-based psycho-emotional support to clients and caregivers. Build and maintain collaborative partnerships with agencies to enhance service delivery and client empowerment (e.g., vocational reintegration, transitional planning). Participate actively in transdisciplinary team meetings, case conferences, and joint care planning with therapists and allied health professionals. Conduct case closures with measurable and sustainable client outcomes, exercising professional autonomy. Identify emerging client and community needs through trend analysis and community engagement. Initiate, plan, lead, and execute outreach efforts to raise awareness and extend services to underserved populations. Engage stakeholders and community partners to co-develop capacity-building initiatives and inclusive support networks. Design, facilitate, and evaluate groupwork interventions for clients and caregivers. Organize and lead psychoeducational training sessions and caregiver support groups to build resilience and enhance caregiving capacity. Offer emotional support and counselling to caregivers to address stress, burnout, and related challenges. Lead and coordinate community-based projects and social work initiatives (e.g., caregiver support programmes), ensuring they meet objectives and are aligned with client and community needs. Apply project management skills including planning, stakeholder management, implementation, monitoring, and evaluation. Engage in regular supervision and reflective practice to enhance professional competencies. Maintain registration and professional development requirements as per the Social Work accreditation board. Participate in training, workshops, and research activities to deepen specialized knowledge and stay updated with best practices. Provide mentorship or supervision to junior Social Workers and Social Work Associates, where applicable.
Recognised Degree in Social Work. Accredited as a Registered Social Worker (RSW). Preferably with minimally 5 years of social work experience in the fields of Early Intervention/ Special Education. Strong interest and passion about working with children with special needs. Good communication and interpersonal skills. Able to work effectively within a multi-disciplincary team.
