Job Detail

"Perform finger prick Point of Care (POC) glycated haemoglobin (HbA1C) test and blood pressure (BP) for visiting residents. Provide individual counselling depending on POC HbA1C test result, blood pressure reading. Provide follow up to residents’ post POC HbA1C testing. Assist in referring residents to Primary Care Network(PCN) / General Practitioner (GP) or Polyclinic for follow-up. Assist visitors in Do It yourself (DIY) Eye check (Snellen Chart) and BMI measurement if necessary. Perform daily equipment readiness check to ensure that it is useable and in good condition. Data management, including screening results, questionnaires and forms. Inventory management of medical consumables, including ordering of medical consumables. Provide general health promotion advice on healthy eating, healthy lifestyle and encourage physical activity, recommendation on vaccination and age appropriate health screening tests. Provide diabetes counselling and education when necessary. Advise residents on relevant information on health resources and connect them to community health resources. Participation in outreach activities / events in the community Organise and facilitate group activities at CHRC Any other duties as assigned"
