Join us as a Case Executive and be part of our CREST-PDS team. You will need to Provide case management support to clients who are newly diagnosed with dementia and their caregivers Provide care coordination and service linkage to clients with dementia and their caregivers, with the appropriate resources and support including caregiver support Participate in multi-disciplinary case discussions with hospital professionals relating to the well-being of clients who are diagnosed with dementia Conduct regular home visits to monitor the well-being of the caregiver and client with dementia Support the evaluation and documentation of the programme Support other ad-hoc assignments as assigned by CREST-PDS Lead/Department Head
You should Have good knowledge of the geriatrics and community services Be proficient in Google Suite applications Have strong communication and stakeholder management skills Be passionate about community services and be service-oriented Have good assessment and decision-making skills Be meticulous and enjoy working independently
