Your job includes but is not limited to the following: Monitor residents' wellbeing and care plan through regular check-ins. Coordinatate health and social care by liaising with various stakeholders within the integrated community care team. Implement changes to care plans developed with residents, in consultation with SAWBCs/ WBCs. Maintain updated documentation of care plan, post-intervention follow-ups, and engagements with resident's and/or caregivers. Assist in screening residents for health and/or social care needs such as administering physical & functional ability tests, basic vital sign measurements, simple questionnaires to understand residents' financial situation, screening residents' home environment and etc. Share information on suitable community resources and services for residents to rely on. Support team with community outreach and engagement efforts.
