The Health Coach will be stationed at Community Health Posts in the community to engage and support residents in improving their health. He/she will also support the planning and implementation of population health programmes, activities and events.
"1) Conduct basic care needs assessment and engage residents to raise awareness of health status and establish health promoting goals. 2) Provide lifestyle interventions to change and sustain lifestyle-related behaviours with the intent of reducing health risks or improving self-management of chronic conditions and increasing health-related quality of life. 3) S/He may require guidance and/or supervision for assessment and management of clients/residents with higher complexity. 4) Collaborate with other members of the Community health Team (CHT) to facilitate holistic care planning and service provision."
